Encouraging and supporting patients living with long-term conditions to self-care.
Full Abstract
This article outlines a holistic approach to supporting patients with long-term conditions to self-care. This approach promotes the self-care ethos to nursing staff, encourages patient involvement and improves overall health and well-being. It also incorporates use of the booklet Your Health, Your Way--A Guide To Long-Term Conditions and Self-Care (Department of Health, 2009). Both authors were involved in developing this resource.
